This provides information about you and your animal which needs to be emailed or sent directly to your veterinarian to gain their consent prior to commencing physiotherapy.  I will need to see the completed form with vet signature before booking your appointment.

This is a legal requirements under the  Veterinary Surgeons Act 1966.
